CHECKLIST OF REQUIREMENTS

  1. Duly filled up Amendment Form
  2. Official Receipt of P100.00 (Amendment Fee )
  3. Dully filled up Clearance clearance form with approval from Zoning, OBO and ENRO
  4. Barangay Business Clearance for New Address
  5. Contract of Lease if place is rented, if not, Affidavit of Non-Rental

CLIENT STEPS

  1. Submit the requirements to Business Permits and Licensing Office
  2. Secure New FSIC from BFP and New Sanitary Permit to Operate from CHO and submit to BPLO

(If Mayor’s Permit of current year has been issued, client needs to surrender the original copy)

CHECKLIST OF REQUIREMENTS

  1. Death Certificate of the deceased (Probational and Original)
  2. Burial Permit Fee – 50.00

CLIENT STEPS

  1. Present the original or temporary/provisional death certificate to the person in charge at the BPLO.
  2. Receives the death certificate.

CHECKLIST OF REQUIREMENTS

Need to upload the following upon Online Application :

  1. DTI for Single Proprietorship (original) SEC Registration with Articles of Inc. for Corporation / Association/Partnership/OPC (original) with updated GIS from SEC if the SEC registration is not recent CDA Registration for Cooperatives (original)
  2. Notarized Contract of Lease if Place of Business is Rented If not renting – Notarized Affidavit of Non-rental (sole prop) / Secretary’s Certificate (corp.)/ Proof of ownership if the property is owned
  3. Franchise Agreement/ Certificate of Dealership / IPO Registration if the business will use another trade name rather than its own

3. Simultaneous Online clearance/approval from concerned regulatory offices and agencies.
Start tracking your application by going to ebpls.bacolodcity.gov.ph , and click TRACKING.

4. The Mayor’s Permit will be sent to client’s registered email address once clearances and payment are completed.

CHECKLIST OF REQUIREMENTS

  5. Extra-Judicial Declaration of Heirship
  6. Waiver of rights in favor of the New Owner

  1. Client presents the death certificate to be cremated.
  2. Client receives the cremation permit.

CHECKLIST OF REQUIREMENTS

  1. Death Certificate of the person to be exhumed
  2. Payment of 75.00
  3. Payment of Inter of Bones or Ashes 75.00

CLIENT STEPS

  1. The client presents the original or certified true copy of the death certificate for exhumation.
  2. Client receives the exhumation permit.

CHECKLIST OF REQUIREMENTS

  1. Official Receipt from CTO – Cashier

CLIENT STEPS

  1. Request for a certification at the CMO-Permits and License Division.
  2. Pay at the CTO-License Division and returns at the CMO-Permits and License Division to present the OR as proof of payment
  3. Accepts the Mayor’s Certification.
